• Record Release Form - if your child has any dental records from another dental provider, such as radiographs or past treatment

Is your child an existing patient? 

• Update Patient Information  - if there are any changes to address, phone number, parent/guardian information, etc.
• Update Insurance - if your insurance has changed since last visit
• Consent - Authorization of Dental Treatment For a Minor if someone other than a parent/guardian is bringing child/ren to appointment (NYS law requires that any patient under the age of 18 is accompanied by an adult)
